Compass Associates is delighted to be working with a leading independent provider of Community Complex Care services, specialising in Paediatric and Adult care. Our client is rapidly expanding and is recognised for their exceptional services, which include care for conditions such as Brain Injury, Cerebral Palsy, Multiple Sclerosis, and Motor Neurone Disease.
We are currently recruiting a Clinical Case Manager for North London to oversee a caseload of Paediatric and Adult community complex care packages. In this role, you will be responsible for training and mentoring Regional Nurses and Healthcare Assistants (HCAs) in key competencies such as tracheostomy care, PEG feeding, and ventilator management. You will also collaborate with commissioners to grow the service by taking on new packages while maintaining strong relationships with existing clients and ensuring that their care remains of the highest quality.
Role Responsibilities:
As a Clinical Case Manager, your key responsibilities will include:
- Managing a caseload of Paediatric and Adult community complex care packages.
- Training and mentoring Regional Nurses and HCAs in clinical competencies such as tracheostomy care, PEG feeding, and ventilator management.
- Collaborating with commissioners to expand care packages and support organisational growth.
- Developing and maintaining strong relationships with clients, their families, and the wider clinical team.
- Ensuring the delivery of high-quality, person-centred care through regular reviews and feedback.
- Acting as a point of contact for clinical guidance and support for the care team and clients.
What We’re Looking For:
- Must live in North London.
- Hold a valid UK driving licence and have access to a car.
- A minimum of 2 years’ post-qualification experience.
- RGN, RSCN, or RCN qualified.
- Experience working in a Complex / Critical Care setting (e.g., ICU, ITU, CCU, HDU).
- Trained in tracheostomy care, ventilator management, and PEG feeding.
- Excellent communication skills and the ability to build effective relationships with clients, families, and colleagues.
